Surrogate Family Care is a Family-Owned and Operated Home Care Agency that assists Private-Pay AND Veteran patients throughout NYC, Queens, Brooklyn, Staten Island, Long Island, Westchester, Putnam & Dutchess County!

(On the Road) Per Diem:

• Patient Case Openings ( Assessments, Medication Lists, Creating a plan of care, Educating family and caregiver of care plan)
• Post Hospitalizations Visits (Changes in condition, medications, plan of care, MD orders)
• Revisits (Changes in plan of care, Medications, MD orders)
• IV Infusions (Certification is a plus)

MILEAGE IS COVERED!!!! WEEKLY PAY!!!!

Experience and Qualifications:

• RN License
• Retired nurses welcome!!!
• Healthcare Agency Experience a plus
• Authorization to work in the U.S
• Reliable Transportation
• Annual Employment Physical
• Positive and motivated mindset!
