Public Health Nurse

Summit County Public Health is seeking a Public Health Nurse to work full-time (35 hours per week), Monday through Friday. This position provides case management and support for families enrolled in the Children with Medical Handicaps (CMH) program, a health care program through the Ohio Department of Health.
The CMH program connects families of children with special health care needs to a network of quality providers and helps them obtain payment for the services their children need. In this role, the Public Health Nurse processes referrals, coordinates services, and helps families navigate resources that support the diagnosis and treatment of medically eligible conditions.
Key Responsibilities
- Provide comprehensive case management and service coordination for families enrolled in the Children with Medical Handicaps (CMH) program.
- Assist families in accessing appropriate sources of payment for medical services and coordinate referrals to ensure timely diagnosis and treatment.
- Collaborate with healthcare providers and community agencies to increase access to care for children with special health care needs.
- Conduct quality assurance activities to identify gaps in services, help establish standards of care, and determine unmet needs of children with handicaps and their families.
- Assess, plan, and implement nursing strategies to improve the health of individuals and populations.
- Complete goals and objectives for assigned grant-funded programs and contribute to proposal development when needed.
- Maintain accurate clinical documentation and records using electronic medical records and follow program compliance requirements.
- Monitor and evaluate program performance, identify barriers, and recommend improvements.
- Maintain cooperative relationships with internal and external partners, community members, and families served.
- Participate in staff meetings, trainings, community events, and other agency functions.
- Be available for emergency response, including 24/7 availability, as needed in accordance with agency policy.
Qualifications
- Bachelor's degree in Nursing (BSN) required.
- Active Registered Nurse (RN) license in the State of Ohio.
- Minimum of two years of nursing experience (preferred).
- Previous public health or case management experience (preferred).
